Discharge Process

A new discharge process is in practice as of September 7th, 2023; whereas we used to put in the “ED Discharge” order which would have a yellow house icon pop up, starting on the date previously referred to, the process is:

  1. Patient is ready to be discharged as determined by the resident/APP
  2. The resident/APP places the “Discharge Request Pending Physician Agreement” order. This will put up the RED house icon on the tracking board.
    • This action replaces the “Needs Attending Approval” comment we would normally place on the tracking board
    • This order should not be placed until the patient is ready to be discharged
    • Continue to use the “DCI Done _rx” comment to let the RN know that DCI is done
  3. Let the attending know that the patient is ready to be discharged and pending their approval
  4. The attending will place the “Discharge” order (the same one we used to use to ‘put the house up’), this will put up the YELLOW house with the brown roof icon
    • This is equivalent to the “Attending Approves” comment on the tracking board

To make things easier – FAVORITE the “Discharge Request Pending Physician Agreement” order; residents/APPs will still be able to put in the attending “Discharge” order if necessary/communicated by the attending
